Alongside your ongoing supports, Support at Home includes dedicated short-term pathways for particular moments in life — regaining independence after a setback, and care at the end of life.
Most in-home support is steady and ongoing. But there are two moments where more intensive, time-limited care makes a real difference: when someone is trying to recover their independence after an illness, a fall or a hospital stay — and when someone is nearing the end of their life and wants to be at home.
Support at Home funds both of these as separate pathways, with their own budgets and their own focus. Your eligibility for either is determined through assessment.
