Notes about the support provided, and any feedback you give us.
How we collect and use it
Wherever possible we collect information directly from you. With your consent we may also collect it from people involved in your care — such as a family member, your GP, My Aged Care or an assessor.
We use your information to plan, deliver and coordinate your care, to keep you safe, to meet our legal and funding obligations, and to improve our service. We do not sell your information.

Stored securely
Held in secure systems with access controls.
Need-to-know access
Seen only by the people involved in your care.
Trained, bound staff
Handled by staff trained in privacy and bound by confidentiality.
Kept only as needed
Retained as required, then securely destroyed.
Breach response
A clear process to act quickly if information is ever at risk.
Sharing your information
We share your information only with your consent, or where permitted or required by law — for example, with your GP or care team to coordinate your care, or in a genuine emergency to protect your health and safety. We do not routinely disclose your information overseas.

Ask us to correct anything that is wrong or out of date.

Ask how your information is used and who it is shared with.
